Job Introduction

We are looking for a dedicated Field Based Care Manager to oversee a designated area and ensure customers receive safe, person-centred, high-quality care. You will lead a team of Care and Support Workers, manage performance, and maintain strong compliance standards.

THIS ROLE IS FOR THE TW16 AND TW17 AREA.

Key Duties

  • Complete and review person-centred care plans and risk assessments.
  • Carry out initial assessments, regular customer reviews, and respond to concerns or complaints.
  • Lead, supervise, and support Care and Support Workers, including training, spot checks, and performance management.
  • Manage rotas using our electronic rostering system, ensuring accurate call times and appropriate travel time.
  • Take part in the on-call rota, supporting emergency cover and operational needs.
  • Ensure customers are kept informed of any changes to their care.
  • Maintain accurate records and ensure all policies, procedures, and safety standards are followed.
  • Attend meetings, support service development, and contribute to continuous improvement.
  • Provide direct care when required and uphold excellent communication across the service.

This role requires strong leadership, excellent organisation, and a commitment to delivering outstanding care to our customers.

  • Ability to organise staff meetings/ evening events / appreciation events
  • Care and Support Workers meeting for the branch
  • Performance manage underperforming staff and follow the legal process with the HR department
